Trigeminal neuralgia is a chronic pain condition and one of the most severe neurological disorders affecting the face. Also known as tic douloureux, it is sometimes called the 'suicide disease' due to the severity of the pain.
Trigeminal neuralgia affects around 8 in every 100,000 people in the UK, with a higher prevalence in women than men, and it typically begins between the ages of 50 and 60.
The trigeminal nerve, the fifth cranial nerve, transmits sensory information from the face to the brain. The most common cause of trigeminal neuralgia is compression of the trigeminal nerve by an abnormal blood vessel, although it can also occur as a symptom of multiple sclerosis. In rare cases, no cause can be identified. This is referred to as idiopathic trigeminal neuralgia.
Trigeminal neuralgia causes stabbing, shooting, or burning sensations, usually on one side of the face. This intense facial pain can feel electric shock-like and lasts anywhere from a few seconds to several minutes. Everyday activities such as speaking, eating, or exposure to a light breeze can trigger pain attacks.
There are no specific diagnostic tests to diagnose trigeminal neuralgia. Diagnosis is based on symptoms. Most people with trigeminal neuralgia will be prescribed medicine to help control their nerve pain. Other treatment options include surgical procedures such as stereotactic radiosurgery, also called Gamma Knife radiosurgery, which uses targeted radiation to reduce or block pain signals from the nerve, and microvascular decompression, which can relieve pressure on the nerve.
Minimally invasive procedures used to treat trigeminal neuralgia include glycerol injections and balloon compression, which also work by deliberately injuring the nerve to stop it from sending pain messages.
